Lp-PLA2 (Lipoprotein-Associated Phospholipase A2)

Lp-PLA2 (also known as the PLAC test) is an enzyme produced by macrophages within arterial plaque. Unlike hsCRP, which reflects systemic inflammation from any source, Lp-PLA2 is specific to vascular inflammation and actively involved in the atherosclerotic process.

Elevated Lp-PLA2 indicates active inflammation within arterial walls and has been validated as an independent predictor of coronary heart disease and ischemic stroke, even in patients with normal LDL cholesterol. The FDA has cleared it as a cardiovascular risk marker.

Lp-PLA2 is particularly valuable when hsCRP is normal but cardiovascular risk factors persist, as it can identify vascular inflammation that systemic markers miss.
